Summary
Honda (American Honda Motor Co.) is recalling specific 2003 Acura 3.2CL, 2013-2016 ILX, 2013-2014 ILX Hybrid, 2003-2006 MDX, 2007-2016 RDX, 2002-2003 3.2TL, 2004-2006, and 2009-2014 TL, 2010-2013 ZDX and 2001-2007 and 2009 Honda Accord, 2001-2005 Civic, 2003-2005 Civic Hybrid, 2001-2005 Civic GX NGV, 2002-2007 and 2010-2011 CR-V, 2003-2011 Element, 2007 Fit, 2002-2004 Odyssey, 2003-2008 Pilot, and 2006-2014 Ridgeline vehicles. The affected vehicles received a replacement driver air bag inflator as part of a previous Takata inflator recall remedy or a replacement driver air bag module containing the same inflator type as a service part. Due to a manufacturing error, in the event of a crash necessitating deployment of the driver frontal air bag, these inflators may explode.
Remedy
Honda will notify owners, and dealers will replace the driver’s air bag inflator with an alternate inflator, free of charge. The recall began April 10, 2019. Honda owners may contact customer service at 1-888-234-2138. Honda’s number for this recall is O41. Acura owners may contact customer service at 1-888-234-2138. Acura’s number for this recall is U40.

BACKGROUND
In certain vehicles, the replacement driver’s (front) airbag inflator (from either a previous recall repair or an airbag module replacement) could produce excessive internal pressure during airbag deployment. If an affected airbag deploys, the increased internal pressure may cause the inflator casing to rupture. Metal fragments could pass through the airbag cushion material, possibly causing injury or fatality to vehicle occupants.
NOTE
If the VIN status inquiry indicates that the vehicle is subject to any open safety recalls, including a front passenger’s airbag inflator campaign/recall, make sure to also complete those recall repairs before returning the vehicle to the client. If you are unable to complete any open recall, advise the client of the recall status and when the recall could be completed.
You may receive a different part number than ordered, and the inflator may appear different. As a result, there may be different installation instructions. Make sure you review this service bulletin for installation instructions and details about model applicability.

NOTES
- The metal snips have smaller jaws compared to regular tin snips to give you better mechanical advantage and allow the snips to fit in the tight area around the airbag.
- Make sure you use these snips to minimize the amount of debris created. Never allow any debris to fall into the airbag while the inflator is out of the airbag module.
- Replace the snips when they are dull. Dull snips may contribute to the horn plate being bent or creating small pieces of metal that could get into the airbag.

WARRANTY CLAIM INFORMATION
NOTES
• To avoid a warranty claim being charged back to your dealership, the defective, undeployed inflator must be
returned to its supplier. Follow the shipping instructions in PIB A15-0006, Inflator Kit Return Process.
• The replacement inflator part number and serial number are printed on the label that should be attached to the
RO.
• To avoid non-payment of warranty claims, the new airbag inflator part number and serial number must be included
in each claim. For more information, refer to step 5 of REPAIR PROCEDURE.
• The serial number is encoded into the bar code printed on the label inside of the box and on a label attached to the
outside of the box. If your dealership uses a scanner, you can scan the serial number from either place into the
claim.
• The part number is encoded into a barcode attached to the outside of the box. If your dealership uses a scanner,
you can scan the part number into the claim.
• If you confuse the serial number and part number when entering the claim information, your claim will
automatically be rejected.

REPAIR PROCEDURE
NOTE:
• If the VIN status inquiry indicates that the vehicle is subject to any open safety recalls, including the front
passenger’s airbag inflator, make sure to also complete those recall repairs before returning the vehicle to
the client. If you are unable to complete any open recall, advise the client of the recall status and when the
recall could be completed.
• Make sure you have the anti-theft code for the audio and navigation system (if applicable), then write down the
frequencies for the radio’s preset buttons.
• Be careful not to damage any parts when replacing the inflator, and follow the procedure exactly.
• If, before removing the airbag module, you suspect that it may be the incorrect airbag for the vehicle or a counterfeit,
do not remove the airbag. Contact your DPSM for assistance.
• Make sure you have reviewed the SRS Precautions and Procedures in the service information before doing the
REPAIR PROCEDURE.
1. Turn the ignition switch to the ON (II) position and check if the SRS indicator comes on for about 6 seconds, then
turns off.
• If the indicator comes on, then turns off after about 6 seconds, go to step 2.
• If the indicator does not come on, or if it stays on, contact your DPSM for assistance.
2. Disconnect the battery negative cable and wait at least 3 minutes before continuing.
3. Remove the driver’s airbag module. Refer to the applicable vehicle’s service information.
4. Place the airbag module, face-down, on a clean shop towel.
NOTE:
• The orientation of the airbag module is critical because if the inflator is installed incorrectly, the harness will be
too short when you reinstall the airbag module.
• The brackets between the various models are similar but may differ from the example shown.

5. Peel the label located inside the inflator box from its backing and attach it to the RO. For the warranty claim to be
paid, the replacement inflator part number that you received must be included in the warranty claim.
NOTE:
• The replacement part number is printed on the top part of the label and the serial number is printed on the
bottom. Make sure you attach the label to the RO and give it to the warranty clerk.
• There may also be a colored sticker located in the kit, ignore it because it is not used.
• Not all inflator kits have a label included. In those cases, write the serial number and part number from the side
of the inflator box on the RO. The part number always starts with 047.
6. If there is no horn plate installed, go to step 7. If there is a horn plate installed, there are several ways to access the
inflator. There are two types of horn plates that can be removed and one type of horn plate cannot be removed
(2007–12 RDX, 2009–14 TL, 2010–13 ZDX) and may need to be cut.
6.1. Type 1 – 2003 3.2CL, 2002-03 3.2TL, 2003-06 MDX
6.1.1. Remove the four horn plate mounting bolts, the mounting bolt felt washers, the horn plate, and remove
the felt tape covering the lower two inflator locknuts. The felt washers and felt tape will not be reused.

6.2. Type 2 – 2013-16 ILX, 2013–14 ILX Hybrid, 2013-16 RDX
6.2.1. Evenly loosen the three bolts using a 1-inch socket.
NOTE
Do not lose any of the small rubber washers or springs for the horn plate; they are not available
separately.
6.2.2. Remove the 3 bolts. Make sure you do not drop or lose the 3 black rubber washers.
6.2.3. Remove the horn plate. Be careful that the 3 springs stay attached to the horn plate. If the springs
come loose, reinstall as needed and set aside the horn plate and components until reassembly.

6.3. Type 3 – 2007–12 RDX, 2009–14 TL, 2010–13 ZDX
NOTE
If the bracket has been previously cut, go to step 7.
6.3.1. Using a marker, draw a line on the horn plate as shown.
6.3.2. Using the snips, cut along the line drawn.
NOTE
Always use sharp snips. If they are dull and you twist or wiggle them back and forth, you may warp the
horn plate, which would require a new airbag module.

6.3.3. Take the felt tape and cut it into two pieces. Apply to the cut surface for protection.
NOTES
• Do not file the metal edges.
• Make sure the felt tape does not interfere with the horn plate or horn plate springs.
6.3.4. Measure the space between the two copper contacts on the horn plate as shown. The space should
be between 1.1 and 1.9 mm.
NOTE
There is a copper contact on each side of the airbag module.
• If the spaces are within specification, write the measurements on the side of the horn plate.
• If either of the spaces is out of specification, try to correct it. If the space is too large, use a pair of
pliers to close the space, or if the gap is too small, use a flat headed screwdriver to open it up so
that the copper contacts are within specification. Write down the measurement on the side of the
horn plate.

7. Using needle-nose vise grips or a 3/8 in. 6-point socket, remove the four locknuts from the inflator. The locknuts will
not be reused.
NOTE
Do not remove the inflator from the airbag module until you complete step 8 .
8. Use shop air to clean any debris that may be on the inflator, then remove the inflator from the airbag module.
NOTES
• Do not allow any debris to enter the inflator opening in the airbag.
• Do not deploy the inflator. The inflator must be returned to its supplier in the box the new inflator came in. Follow
the shipping instructions supplied with the inflator kit. If you have any questions about shipping, call Warranty
Parts Inspection (WPI) at 937-642-2737.

9. Turn over the airbag module (inflator side down), and shake it to remove any debris from the inflator opening.
10. Install the new inflator in the airbag module.
NOTE
There may be some minor surface rust or corrosion on the new inflator; this is OK.

10.2.1. Place the inflator into the opening with the inflator harness toward the bottom of the airbag module. Do
not force it into the airbag module.
10.2.2. Using a pair of pliers, gently pull up on each stud up while pushing the inflator module down until the
airbag is properly seated within the airbag module, then go to step 11.

10.3. Type 4 – Check your part number against the ones listed for each installation procedure. Use the
correct installation procedure for the part you received.
NOTE
Refer to Driver’s Inflator Type 4 in the video.
10.3.1. Place the inflator into the opening with the inflator harness toward the bottom of the airbag module.
10.3.2. Install the bracket onto the studs with the stamp LOWER at the bottom of the airbag module.
10.3.3. Make sure the three tabs and notches are properly aligned, then go to step 11.
NOTES
• The tabs and notches are designed to align in one direction only. If the tabs and notches are not
aligned, the inflator and bracket are not installed correctly.
• Make sure the harness comes out through the center opening. Do not install the bracket over the
harness wires.

11. Install and torque the new locknuts to 3.9 N·m (2.8 lb-ft, 34 lb-in).
12. If applicable, install the horn plate.
12.1. Type 1 Horn Plate
12.1.1. Cover the lower two inflator locknuts with new felt tape (if applicable).
12.1.2. Install the horn plate with new felt washers on its mounting bolts. Torque the mounting bolts to 3.4 N·m
(2.5 lb-ft, 30 lb-in), then make sure the horn plate moves freely.

12.2.1. Make sure the three springs are mounted to the back side of the horn plate as shown.
12.2.2. Make sure the rubber washers are installed on their respective bolts.
12.2.3. Install the horn plate on the airbag.
12.2.4. Install the 3 bolts and evenly tighten to 4.0 N·m (3.0 lb-ft, 36 lb-in).
NOTE
Do not over-tighten the bolts because you can break the studs and you will have to replace the
airbag module.
13. Install the driver’s airbag module using new Torx bolts from the kit and torque the bolts to 9.8 N·m (7.2 lb-ft). Refer
to the service information.
14. Reconnect the battery negative cable and do the following:
• Enter the anti-theft codes for the audio system and the navigation system (if equipped).
• Press and hold the audio unit power button to exit the anti-theft mode.
• Enter the radio presets.
• Set the clock (on vehicles without navigation).
15. Turn the ignition to ON and make sure the SRS indicator comes on for about 6 seconds, then goes off.

16. Give your warranty clerk the RO that includes the replacement inflator serial number and replacement part number.
For the warranty claim to be paid, the replacement inflator part number must be included in the warranty claim.

If an affected airbag deploys, the increased internal pressure may cause the inflator to explode. In the event of an inflator explosion, metal fragments could pass through the airbag cushion material and into your vehicle interior at high speed, potentially causing serious injury or death to you or others in the vehicle.
WHAT WILL ACURA DO?
Acura will replace the driver front airbag inflator for free.The driver front airbag module, which is located in the steering wheel, is comprised of the inflator and an airbag module cover (see illustration on the right). The inflator is the only component that will be replaced.
WHAT SHOULD YOU DO?
Replacement inflators are available so please call any Acura dealer and make an appointment to have your vehicle repaired for free. Once you make an appointment for your vehicle, be advised that the complete repair process may take approximately 30 minutes. However, your dealer can provide you with a better estimate of the overall time for this repair visit, so it may be necessary to make your vehicle available for a longer period of time. A FREE loaner car, towing service or other alter-
native transportation may be available to assist with any inconvenience this repair may cause. If you are not the only driver of this vehicle, please advise all other drivers and passengers of this important information!

El inflador de reemplazo de la bolsa de aire delantera del lado del conductor podría producir presión interna excesiva una vez que se active. Honda launched a regional safety improvement campaign NHTSA ID 14V-351 and began using PSDI-5D inflators as recall remedy parts.
March 2015
As the scope of the non-desiccated PSAN airbag inflator recalls expanded, Honda began sourcing recall remedy airbag inflators from alternative manufacturers to supplement the supply of replacement parts.
July 2016
Honda stopped ordering, and instructed dealers to stop installing, the PSDI-5D inflator as a recall remedy part.
March 21, 2018
Honda was informed of a January 19, 2018 crash in which it was alleged that the driver’s airbag inflator had ruptured during airbag deployment.

An initial rupture incident report was submitted to NHTSA per Standing General Order 2015-01A.
April 10, 2018
A joint field inspection of the subject vehicle performed by the airbag inflator supplier, NHTSA, and Honda confirmed that the driver’s airbag inflator had ruptured. The inspection also confirmed that the ruptured airbag inflator was a PSDI-5D that was installed in the subject vehicle as a recall remedy part. A final rupture incident report was submitted to NHTSA per Standing General Order 2015-01A.
Mid-April – early February 2019
Honda began an investigation of the ruptured replacement airbag inflator in collaboration with the airbag inflator supplier and NHTSA. As part of this investigation, the parties sourced similar PSDI-5D replacement parts from warehouse inventory and initiated DOE testing. As PSDI-5D replacement parts were collected from the field, including airbag inflators produced in the same timeframe as the ruptured airbag inflator, DOE testing and analysis of those inflators also occurred. Ultimately, analysis of the ruptured replacement airbag inflator, along with the field inflators, did not result in a conclusive determination of the factors that may contribute to the over-pressurization observed in these inflators. However, all available evidence points towards a manufacturing process anomaly.

Honda met with NHTSA and shared the status of the investigation.
March 6, 2019
Honda determined that a defect related to motor vehicle safety existed and decided to conduct a safety recall.
As of March 6, 2019, Honda has received one injury claim related to this issue.

Air Bag Inflator Rupture

Dated opened: February 24, 2015
The Office of Defects Investigation (ODI) opened PE14-016 in June 2014 based on six inflator rupture incidents involving consumer owned vehicles produced by five vehicle manufacturers. All six vehicles were operated in Florida or Puerto Rico at the time of the rupture and for the majority of their service life, and were equipped with inflators produced by Takata, a tier-one supplier of automotive air bag systems.
During the course of PE14-016, ODI determined that five additional vehicle manufacturers used inflators of a similar design and vintage also supplied by Takata. No evidence of field failures was found in vehicles produced by these five additional manufacturers. Nonetheless, at ODI’s insistence, all 10 vehicle manufacturers initiated a regional recall within approximately two weeks of the opening of the investigation. The regions recalled initially included Florida, Puerto Rico, Hawaii, and the U.S. Virgin Islands, areas with high absolute humidity and climatic conditions believed to be a significant factor in the inflator ruptures. As part of the recall actions, inflators removed from remedied vehicles are to be returned to Takata for testing.
Takata’s initial test results on passenger inflators from remedied vehicles indicated a much higher than anticipated rupture frequency for inflators returned from Florida. Accordingly ODI requested all 10 manufacturers expand the regional recalls for passenger inflators to include other geographic areas where high absolute humidity conditions exist, including the Gulf States and other coastal areas. Takata’s testing of the passenger inflators to date continues to indicate this geographic area as having the highest risk, with no ruptures occurring from inflators returned from outside the expanded recall regions. During PE14-016 four additional passenger inflator field events occurred, all in vehicles from the same expanded geographic region.
Also during PE14-016 four additional driver inflator field events occurred including two in vehicles from regions not known for high absolute humidity, specifically California and North Carolina. Accordingly, ODI requested all five of the affected vehicle manufacturers currently using the subject Takata driver inflators expand to nationwide recalls. Significantly, neither of the affected vehicle manufacturers or Takata provided any explanation to account for these two driver air bag inflator ruptures outside the area of high absolute humidity. Takata testing of returned driver inflators indicates a lower rupture frequency as compared to passenger inflator testing. All test ruptures reported by Takata to date have occurred on inflators returned from high absolute humidity areas.
The investigation now includes all manufacturers and vehicles known to be affected at this time. ODI’s investigation will focus on, among other things, root cause analysis, other potential defect consequences, identification of affected vehicles scope, and adequacy of the remedy.
